Clear aligners are a series of thin, transparent trays made to fit your teeth exactly. Each one is very slightly different from the last, and wearing them in order moves your teeth a fraction of a millimeter at a time. You change to the next tray roughly every one to two weeks, and over months the sequence walks your teeth from where they are to where they should be.
The appeal is obvious: nobody can see them from across a table, you take them out to eat whatever you like, and you brush and floss normally instead of threading around wires. The part people underestimate is that they only work while they are in your mouth. Twenty to twenty-two hours a day is not marketing — it is the condition the whole treatment plan is calculated on.
Aligners are excellent for crowding, spacing and mild to moderate bite problems, which covers most of what walks through our door. They are not the right tool for everything. Some bites still need fixed braces, and we would rather say so at the consultation than start a case that quietly underdelivers for a year.
Most adults asking about aligners fall into the first three. The scan at your consultation is what tells us which category yours is, and whether aligners are the right tool at all.

The scan is turned into a plan showing your teeth moving from where they are now to the finished position. You look at that, and the written cost, before agreeing to anything.

Your first trays are fitted and, in most cases, small tooth-colored attachments are bonded to a few teeth. They give the aligner something to grip so it can tip and rotate teeth rather than just push them.
Most cases 6–18 months
You change trays roughly every one to two weeks and see us periodically to check progress. This stage is almost entirely down to you — the plan assumes 20 to 22 hours a day, and the timeline stretches if the wear time does not happen.
Final weeks, then ongoing
Most cases finish with a short set of refinement trays to tidy the last details. Then retainers, which are not optional — teeth drift back for the rest of your life if nothing holds them.

The day-to-day
None of this is difficult. It is just worth knowing before you start rather than discovering in week two.
Aligners are for wearing, not chewing. Food gets trapped, and hard food can crack a tray. Take them out, eat, brush or rinse, put them back.
Coffee, tea, soda and wine will stain the trays and the sugar sits against your teeth under plastic. Coffee in particular yellows aligners within days.
A slight lisp on the first tray of a new set is normal and your tongue adapts within a week. Reading aloud for ten minutes speeds it up more than anything.
Most cases need tooth-colored bumps bonded to a few teeth. Up close you can see them; across a room you cannot. They come off at the end.
Tightness on day one of a new aligner means it is working. Changing trays at bedtime means you sleep through the tightest part.
Rinse in cool water and brush gently with a soft brush. Never use hot water — it warps the plastic and a warped tray no longer fits your plan.
Go back to your previous tray and call us. Keep the last one you finished for exactly this reason rather than throwing it away.
Whitening mid-treatment gives uneven results because attachments cover part of each tooth. Straighten first, whiten after, then match any new work to the final shade.
Aligners are a good treatment and most patients are delighted with them. These are the four things that decide whether you end up in that group.
This is the single biggest difference between aligners and braces. Braces work whether you are motivated or not. Aligners only work while they are in, and a plan built on 22 hours a day does not survive on 14.
Teeth have memory and drift for the rest of your life. Wearing a retainer at night indefinitely is the price of keeping the result. Anyone who does not tell you this before you pay is selling, not treating.
Severe crowding, large skeletal bite differences and some rotations are genuinely better handled by fixed braces. We would rather refer you or recommend braces than run a case that finishes at eighty percent.
Decay, gum disease and failing fillings are treated first, even when that delays the start. Moving teeth through inflamed gums accelerates bone loss, and that is not a trade worth making for a few weeks.
Aligner treatment is straightforward when someone is actually watching the teeth move. It goes wrong quietly when nobody is.
Decay, gum health and existing work are all assessed before a plan is made. Nothing is diagnosed from photographs you took yourself in a bathroom mirror.

You will be given an estimated timeline at the consultation, based on the scan rather than on a general average — and that timeline assumes you wear the aligners as prescribed.

Realistically it means the aligners are in except when you are eating, drinking anything other than water, or cleaning your teeth. Wearing them less does not usually ruin the result, but it does stretch the timeline, sometimes by months.

Usually yes, with some planning. Crowns and veneers can be moved like natural teeth, though attachments do not bond to porcelain as reliably, which sometimes changes the approach. Implants are different: an implant is fused to the bone and does not move, so it becomes a fixed point the plan has to work around. Any of these is a reason to have the case planned by a dentist who can see all your existing work, rather than remotely.
