Third molars, usually ages 17–25
Wisdom teeth are the last four molars to arrive, normally somewhere between seventeen and twenty-five. When there is room for them and they come through straight, they are ordinary teeth and there is nothing to do. The problem is that for a lot of people the jaw simply ran out of space about ten thousand years ago, and the tooth ends up wedged at an angle against the molar in front of it.
That is when they cause trouble: a gum flap that keeps getting infected, decay on the back of a healthy second molar that cannot be cleaned or filled, pressure and ache along the jaw, or a cyst forming quietly around a tooth that never erupted at all. Any of those is a reason to take the tooth out, and the recovery is easier at twenty than it is at forty-five.
What we will not do is remove four healthy wisdom teeth because it is the default. If yours are through, upright and cleanable, the honest answer is to leave them and keep an eye on them at your regular check-ups — and that is the answer you will get here.
You do not need all of these. One clear reason on the X-ray is usually enough, and none of them at all is a perfectly good reason to leave the tooth alone.
A dull ache behind the last molar that comes and goes, often worse when you are run down or after a long day of clenching.
Most common sign
Pericoronitis — food and bacteria trapped under the gum over a half-erupted tooth. It settles, then comes back, usually at the worst moment.
Recurring Infection
A wisdom tooth sitting at an angle traps plaque against the healthy molar in front. That second molar is often the tooth actually at risk.
Protects the tooth in front
An impacted tooth pushing forward can resorb the root of its neighbor. This one is usually silent until an X-ray finds it.
Found on X-ray
Fluid-filled sacs can develop around a tooth that never came through. Uncommon, but a clear reason to remove it.

Difficulty opening fully, a swollen cheek, or a bad taste at the back of the mouth on one side.

Not all wisdom teeth are the same job. Where the tooth sits decides how it comes out, how long it takes, and what you pay — which is why the scan comes before the quote.

Recovery ranges are typical, not a promise. Lower wisdom teeth generally take longer to settle than upper ones, and how you heal matters as much as how the tooth sat.
Start to finish
Two appointments for most people: one to look and plan, one to do it. Here is the order and roughly how long each part takes.
One visit · about 30 minutes
We look at the teeth and take a scan of the whole area. On lower wisdom teeth the scan also shows how close the roots sit to the nerve running through the jaw, which is the single most important thing to know before starting.
Same visit
You are told which teeth need to go, whether each one is a simple or a surgical extraction, what it costs and how long you should expect to be sore. If you would rather be sedated, that is discussed here, not on the day.
One visit · 30–60 minutes for most cases
Dr. Gandhi numbs the area thoroughly first. A simple extraction takes minutes; an impacted lower tooth takes longer because it is usually sectioned and taken out in pieces, which is gentler on the bone than forcing it whole.
Same visit
Surgical sites are usually stitched, most often with sutures that dissolve on their own. You bite on gauze until the bleeding stops, and we go through the aftercare with you before you leave rather than handing you a leaflet.
Days 1–7
Swelling peaks around the second or third day and then improves. Most people are back at work or school within two to four days after a simple extraction, and take a little longer after a bony impaction.
About a week later
We check the sockets are healing cleanly and remove any non-dissolving stitches. Ring us before that if anything worries you — there is no charge for a phone call.
What the week actually looks like
General guidance for a straightforward removal. Your own instructions come from Dr. Gandhi on the day and take priority over anything written here.
Everything in the first day is about leaving the blood clot in the socket alone. Dislodging it is what causes dry socket, and it is almost entirely preventable.
1. Bite firmly on the gauze for the first 30–45 minutes
2. No rinsing, spitting, straws or smoking for at least 24 hours
3. Cold compress on the cheek, 20 minutes on and 20 off
4 . Soft, cool food; nothing hot and nothing crunchy
5. Rest with your head propped up, and skip the gym
This is usually the worst of it, and it is normal rather than a sign something has gone wrong. Bruising along the jaw can appear now too.
1. Start gentle warm salt-water rinses after meals
2. Keep up the pain relief on a schedule rather than waiting for pain
3. Swap the cold compress for gentle warmth if the jaw feels stiff
4. Still soft food — eggs, yoghurt, soup that has cooled, pasta
5. Do not poke the socket with your tongue or a brush
Swelling settles, opening your mouth gets easier and most people are back to normal routine. Pain that is getting worse instead of better at this stage is worth a phone call.
1. Brush normally elsewhere, carefully near the site
2. Reintroduce ordinary food as it feels comfortable
3. Keep rinsing with salt water after eating
4. Still avoid smoking — it is the biggest risk factor for a bad heal
The gum closes over the socket in the first couple of weeks. The bone underneath keeps filling in quietly for a few months, which you will not notice.
1. Any non-dissolving stitches come out at the review
2. Mild tenderness on chewing can linger a little while
3. Go back to normal cleaning around the area
4. Call us if a socket starts hurting again after it had settled
. Pain that eased and then came back sharply around day three or four, often with a bad taste — this is usually dry socket, and it is quick for us to treat.
. Bleeding that has not slowed after twenty minutes of firm pressure on fresh gauze, twice.
. Swelling that is still increasing after day three, or that spreads towards your eye or neck.
. A fever, or feeling generally unwell rather than just sore.
. Numbness or tingling in the lip, chin or tongue that has not worn off many hours after the appointment.
. Difficulty breathing or swallowing — do not wait for us, go to an emergency room or call 911.
Dry socket is treatable in a single short visit and there is no charge for calling to ask. We would far rather hear from you on day four than on day ten.

At Fine Dental Care a simple extraction is $149 per tooth and a surgical extraction is $249 and up, on our in-house membership plan. Which one applies depends on whether the tooth is fully through or impacted in bone, so we scan first and then give you the per-tooth figure in writing. Having all four done in one appointment usually costs less than four separate visits.
No. If your wisdom teeth are fully through, upright, cleanable and causing no problems, the sensible thing is to leave them and check them at your regular visits. We remove them when there is a reason on the X-ray or in your mouth: recurring infection under the gum, decay that cannot be cleaned, pressure damaging the molar in front, or a cyst around an unerupted tooth.
The extraction itself is done under local anesthetic, so you should feel firm pressure rather than pain. Soreness afterwards is normal and peaks around the second or third day. Most people manage with over-the-counter pain relief taken on a schedule. Sedation options are available if the idea of the appointment is what worries you.
Most people are back to normal routine within two to four days after a simple extraction, and around five to seven days after an impacted lower tooth. Usually yes, and many people prefer it: one appointment, one recovery, one course of time off. Whether it is right for you depends on how difficult the individual teeth are and how you feel about a longer appointment. We will give you both options and the cost of each at the consultation.
Plan on two days for a straightforward removal and three to four for impacted lower teeth. Booking the appointment for a Friday or a Saturday morning gives you the weekend, which is what most of our patients in the Energy Corridor do.
Not for a routine extraction under local anesthetic — you can drive yourself. If you choose any form of sedation, you must arrange for someone to drive you home and stay with you, and we will confirm that with you beforehand.
Soft and cool for the first day: yoghurt, smoothies eaten with a spoon rather than a straw, eggs, mashed potato, soup that has cooled down. Avoid anything crunchy, seedy or very hot, and chew on the other side. Most people are eating fairly normally again within three or four days.
The nerve supplying the lower lip and chin runs close to the roots of lower wisdom teeth, so a period of numbness or tingling afterwards is a recognized risk. It is uncommon, and almost always temporary when it does occur. This is exactly why we take a 3D scan first: seeing where the roots sit in relation to that nerve is what lets us plan the safest approach, and we will tell you honestly if your case carries more risk than average.
